- Dear Mr. Kingsley:
This refers to the inspection conducted July 16 through September 1,1998, at the Quad Cities
.,
Nuclear Power Station. The purpose of the inspection was to determine whether activities
j
- authorized by the license were conducted safely and in accordance with NRC requirements.
,
The NRC conducted routine resident inspections in this period. Based on the inspectors *
. findings, overall operations at the Quad Cities station were conducted in a safe manner. At the
.
conclusion of the inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

While many maintenance activities observed were performed well and the maintenance
- .
backlog was reduced by about 200 work requests, several maintenance errors occurred which

,
affected equipment performance.- One error involved improper use of a circuit board in a
-
- transmitter for the reactor protective system. The error resulted in the transmitter being
Inoperable for 18 days. Other maintenance problems resulted in scheduled work not being
-
performed on time and led to additional effort by operations, maintenance, and radiation
'
' protection personnel to complete the scheduled work.' Efforts to improve schedule adherence
'n
and material condition were underway at the end of the inspection period, but there was room
for significant progress in these areas.
.
One additional problem with operators mispositioning valves occurred this period, but did not
. jeopardize equipment performanca. In both the' maintenance and operation errors,

configuration control probicms were a factor. Additionally, independent verification or second
'
checks failed to correct the initial problems.
