Failure to Follow procedu,re Finding:

Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained for the fira protection program.

Procedure ADM 13-103, Revision 5, "Fire Protection:

Im p a i rme n t Contcol,"

implements procedures for impaired fire protection equipment including dehraded fire doors.

ADM 13-103, Section 4, requires that fire prototion im p a i rmen t control pe rm i t forms be prepared for all fire protection s< stem impairments.

In addition, Section 4 requires that entries be made in the fire protection impairment control log for the impairments.

Conteary to the above, on June 6,

1988, Vire Doors 13261 and 13281 were observed to be standing open.

Neither fire protection im pa i rmen t control permits nor log entries had been completed for those doors.

Reason for Violation:

Fire doors 13261 and 13281 were le f t open by operations personnel without the requited fire protection impairment control permits or log entra.4 due to incorrectly interpreting the requirements of ADM 13-103.

The involved operations personnel believed it was acceptable to leave the doors open without a fire impa i rm e nt permit when in the area per forming Technic al Specification surveillance tests since the operator was fire watch qualified.

Corrective St,egs Which llave_ Been taken and _ Results Achieved:

The "Fire Protec t ion Impairment Control" procedure, ADM 13-103, has been revised to clearly identify the requirement for obtaining a fire impairment pe rm i t any time a fire door is to be left open.

The Operations Supervisor instructed all Operations personnel by memo of the requirement to issue a fire impairment whenever any fire door is le f t open for a7y reason, except normal passage.

Corrective Steys Vhich Will Be Taken To Avoid Further Violations:

In addition to the corrective steps discussed above, the procedure change to ADM 13-103 was placed in operations required reading as required reading entry number 88-160 to rein force the memo sent to all Operations personnel.
