Notification of revision of the Eco Pak Specialty Packaging Overall Quality Assurance Manual No. QA-3 For Protective Shipping Packages and request for amendment to the Quality Assurance Program Approval For Radioactive Packages, Approval No. 0179, Revision 6, Docket Number 71-0179.
Dear Ms. Eng,
We have revised the Eco Pak Specialty Packaging Overall Quality Assurance Manual No. QA-3 For Protective Shipping Packages and also changed the title to "The Columbiana Boiler Company Quality System Description." This Quality System Description will be The Columbiana Boiler Company ("CBC") corporate quality program and be applicable to activities at the CBC facility in Columbiana, Ohio and the Eco-Pak Specialty Packaging (a CBC division) located in Elizabethton, Tennessee.
Please amend the Quality Assurance Program Approval, number 0179, to add Columbiana Boiler Company,200 West Railroad Street, Columbiana, Ohio 44408.
I have included our check in the amount of $640.00 to cover the administrative charges.
