MISSING SIGNS CONTAINING RADIOACTIVE MATERIAL l

Dear Mr. Impink:

1 Pursuant to a telephone conversation between Cathleen L. Stanley of your organization and Pamela J. Henderson of this off;ce on September 16,1998, it is our understanding that you will  ;

provide us with the following information:

1. According to the sign manufacturer, SRB Technologies, they transferred 76 generally licensed signs containing radioactive material for utilization by Penn-Trafford ,

Elementary / Middle School (PTMS). According to PTMS, Westmoreland Electric was  !

contracted to install the signs. Please account for the location of each of the 76 signs by i providing a list identifying each sign by it's respective model number and sodal number l and the corresponding location of the sign, include in your account, the 5 signs that you have previously stated are missing. Please note that one of these signs, model number BX-MP, serial number 181702, was confiscated by the Pennsylvania Department of  ;

Environmental Protection in March 1998. Thus it appears that only 4 signs remain  !

unaccounted.

2. Please provide us with a written narrative documenting the steps that Westmoreland Electric has taken to locate the 4 missing signs. This should include a check of your other projects that might have been on-going concurrent with Penn-Trafford ,

Elementary / Middle School to inquire as to whether any of the 4 missing signs may  !

have been erroneously installed at tnese locations, in addition, the narrative should include any information or reports you may have received concoming generally licensed signs found missing at any other facilities.

3. According to SRB, of the 76 signs they transferred for the PTMS project,38 were shipped in care of Westmoreland Electric to PTMS in Trafford, Pennsylvania and 38 were shipped to a Westmoreland Electric office in Greensburg, Pennsylvania.

Ms. Stanley contends that none of the signs intended for the PTMS project were shipped to Westmoreland Electric offices. She stated that all of the signs were shipped directly to the PTMS site. Please verify this information and if possible, provide documentation to support Ms. Stanley's claim that no signs were shipped to kg your offices.

4. According to a fax dated July 20,1998, you have 18 generally licensed signs in your possession. According to Ms. Stanley, these signs were not intended for the PTMS project. By model name/ number and serial number, please provide a list identifying the location / facility where each of these 18 signs was supposed to be installed. in addition, provide an explanation as to how and why the signs are currently in your possession (located at your office).
5. Since you are not licensed to possess, utilize or distribute generally licensed devices containing radioactive material, you must return the 18 signs to their respective manufacturers / distributors or the signs must be Installed in the facility that is generally licensed to utilize them. Ms. Stanley stated that she would contact the l manufacturers / distributors of the 18 generally licensed signs that you currently have in l your possession to arrange for their retum. Please confirm that these signs have been retumed to their respective manufacturer / distributor.
