As per our discussion on February 4, 1986, enclosed are copies of Form-314,
" Certificate of Disposition of Materials," which you must submit to terminate your nuclear pacemaker license (SNM-1401). Please complete the form indicating your transfer to Medtronics (include their NRC or Agreement State license number) and the transfer date. You may attach Medtronics' receipt of your pacemaker.
Return one copy to USNRC, Material Licensing Section, 799 Roosevelt Road, Glen Ellyn, Illinois 60137, and maintain one copy for your files. Our Licensing people-will amend your license for termination (there is no fee for termination amendments). If you should have further questions, please contact this office.
