APPENDIX A NOTICE OF VIOLATION Gulf States Utilities- Docket No.. 50-458 River Bend Station License No. NPF-40 During an NRC inspection conducted on December 16-20, 1985, a violation of NRC requirements was identified. The violation involved the failure to properly calibrate the containment high range radiation monitors. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1985), the violation is listed below:

A. Technical Specification 4.3.7.5 requires that the primary containment area radiation monitors be electronically calibrated for all decade ranges above 10 R/hr.

-Contrary to the above, the NRC inspector determined on December 19, 1985, that the electronic calibration on primary containment area radiation monitor (IRMS-RE16A), only included every other decade range between 0 and 100 percent full scale.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including

'for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the

-date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
