1. The Nuclear Regulatory Commission (the Comission) has found that:

A. The application for amendment by Portland General Electric Company, et al., (the licensee) dated March 31, 1988 as revised June 27, 1988 complies with the standards and requirements of the Atomic Energy Act of 1954, as amended (the Act), and the Commission's regulations set fcrth in 10 CFR Chapter I; B. The facility will o)arate in conformity with the application, the provisions of tie Act, and the regulations of the Comission; C. There is reasonable assurance (i) that the activities authorized by this amendment can be conducted without endangering the health and safety of the public, and (ii) that such activities will be conducted in compliance with the Commission's regulations; D. The issuance of this amendment will not be inimical to the comon defense and security or to the health and safety of the public; and E. The issuance of this amendment is in accordance with 10 CFR Part 51 of the Comission's regulations and all applicable requirements have been satisfied.

2. Accordingly, the license is amended by changes to the Technical Specifications as indicated in the attachment to this license amendment, and paragraph 2.C.(2) of Facility Operating License No. NPF-1 is hereby amended to read as follows:

(2) Technical Specifications The Technical Specifications contained in Appendices A and B, as revised th 'ough Amendment No.149 . are hereby incorporated in the license. The licensee shall operate the facility in acco dance with the Technical Specifica-tions, except where otherwise stated in specific license conditions.

3. This license amendment is effective as of the date of its issuance.

6.0 ADMINISTRATIVE CONTROLS 6.1 RESPONSIBILITY 6.1.1 The General Manager shall be responsible for overall facility operation and shall delegate in writing the succession to this responsibility during his absence. .

6.2 ORGANIZATIONAL REQUIREMENTS 6.2.1 GENERAL A facility and a corporate organization shall be established for unit operation and corporate management. The facility and corporate organizations shall include the positions for activities af fecting the safety of the nuclear power plant. -

a. Lines of authority, responsibility and communication shall be estab-lished and defined for the highest management levels through intermediate levels to and including all operating organization positions. These relationships shall be documented and updated, as appropriate, in the form of organizational charts, functional descriptions of departmental responsibilities and relationships, and l job descriptions for key personnel positions, c,r in equivalent forms of documentation. These requirements shall be documented in the FSAR.
b. The Vice President Nuclear shall have corporate responsibility for overall plant nuclear safety and shall take any measures needed to ensure acceptable performance of the staf f in operating, maintaining, and providing technical support to the plant to ensure nuclear safety,
c. The Plant General Manager shall be responsible for overall unit safe operation and shall have control over those f acility activities necessary for safe operation and maintenance of the plant.

d. The individuals who train the operating staff and those who carry out

, the quality assurance functions may report to the appropriate facility

! staf f managers; however, they shall have suf ficient or to ensure their independence from operating pressures,ganizational freedom

e. Although radiation protection individuals may report to a facility staff manager for matters relating to radiological health and safety of employees and the public, the radiation protection mani.ger shall have l direct access to the onsite individual having responsibility for overall unit management. Radiation protection personnel shall have the authority to cease any work activity when worker safety is jeopardized or in the event of unnecessary personnel radiation exposures.

ADMINISTRATIVE CONTROLS 6.2.2 FACILITY STAFF The Facility organization shall be subject to the following:

a. Each on duty shift shall be composed of at least the minimum shif t crew composition shown in Table 6.2-1. The minimum shift crew necessary for safe shutdown is five.
b. At least one licensed Operator shall be in the control room when fuel is in the reactor.

c. At least two licensed Operators shall be present in the i control room during reactor start-up, scheduled reactor l shutdown and during recovery from reactor trips.
d. An individual qualified in radiation protection procedures
shall be on site when fuel is in the reactor.

l e. All CORE ALTERATIONS after the initial fuel loading shall l be directly supervised by either a licensed Senior Reactor Operator or Senior Reactor Operator Limited to Fuel Handling who has no other concurrent responsibilities during this operation, i

l f. A Fire Brigade of at least 5 members shall be maintained I onsite at all times. The Fire Brigade shall not include members of the minimum shift crew necessary for safe shutdown of the unit or any personnel required for other essential functions during a fire emergency. g. Administrative procedures shall be developed and implemented to limit the working hours of shift personnel who perform safety .

related functions (i.e., senior reactor operators, reactor operators, health physicists, auxiliary operators), and key maintenance personnel of the unit staff.

Key maintenance personnel of the unit staff are those personnel who are responsible for the correct performance of maintenance, repair, modification or calibration of safety related structures, systems or components, and who are personnel performing or immediately supervising the performance of such activities.

This applies to key naintenance personnel whether on shift or not.

Adequate shif t coverage shall be maintained without routine heavy l

use of overtime. The baseline for determining overtime use will l

be a 40-hour work week when the plant is operating. However, in I

the event that unforeseen problems require substantial amounts of overtime to be used, or during extended periods of shutdown for refueling, major maintenance or major plant modifications, on a temporary basis, the following guidelines shall be followed:

a. An individual should not be permitted to work more than 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> straight, excluding shif t turnover time.
b. d. Except during extended shutdown periods, the use of over-l time should be considered on an individual basis and not for the entire staff on a shift.

Any deviation from the above guidelines must be authorized by the Plant General Manager or, in his absence, Duty Plant Manager, or I higher levels of management, in accordance with established procedures and with documentation of the basis for granting the deviation. Routine deviation from the above guidelines is not authorized.

h. The Shif t Supervisor and Assistant Shif t Supervisor shall each hold a Senior Reactor Operator License. The Control Operator and Assistant Control Operator shall each hold a Reactor Operator License.

'- ADMINISTRATIVE CONTROLS 6.3 FACILITY STAFF OUALIFICATIONS 6.3.1 Each member of the facility staff shall meet or exceed the minimum qualifications of ANSI N18.1-1971 for comparable positions, except for (1) the Radiation Protection Branch Manager who shall meet or exceed the qualifications of Regulatory Guide 1.8, September 1975 and (2) the Shift Technical Advisor who shall have a bachel..s degree or equivalent in a scientific or engineering discipline with specific training in Plant design, and response and analysis of the Plant for transients and accidents.

6.3.2 The Operations Branch Manager shall hold a Senior Reactor Operator License.

6.4 TRAINING 6.4.1 A retraining and replacement training program for the facility staf f shall be maintained under the direction of the General Manager and shall meet or exceed the requirements and recommendations of Section 5.5 of ANSI N18.1-1971 and Appendix 'A" of 10 CFR Part 55.

6.4.2 A training program for the Fire Brigade shall be maintained under the direction of the General Manager and shall meet or exceed the requirements of Section 27 of the NFPA Code-1975, except for Fire Brigade training sessions which shall be held at least quarterly.

6.5 REVIEW AND AUDIT .

6.5.1 PLANT REVIEW BOARD (PRB)

FUNCTION 6.5.1.1 The Plant Review Board shall function to advise the General Manager on all natters related to nuclear safety.
