DAMAGE TO REACTOR PRESSURE VESSEL DURING LOADIMG ON BARGE The Reactor Pressure Vessel slated for Pebble Springs (Oregon) site dropped approximately 10 feet onto the barge it was being loaded on at the Mt. Vernon, Indiana B&W facility. The vessel came to rest on dunnage stacked on the barge deck. Approximately 11 of the instrument nozzles were bent, some as much as 51 degrees.

The vessel was being lifted on the barge by use of a 500 ton crane using two hooks. The brake on one of the hoists became overheated, allowing the bottom end of the vessel to slip at a rapid rate onto the dunnage.

Another 500 ton crar.e using one hook and a speader bar lifted the vessel and returned it to the shop area for any needed repairs.

Hartford Insurance and Bechtel representatives were at the site when the vessel was dropped.

Portland General Electric is sending an NDE specialist to the site to evaluata the extent of damage.

Region V inspector assigned to the Pebble Springs Project will followup with the applicant (no CP or LWA has yet been issued for dl this project).

Region V (San Francisco) received notification of this occurrence by telephone at 9.30 a.m. August 2,1977, from Portland General Electric Company.
