Inquiry No.88-001 This refers to a telephone inquiry by Steven Courtemanche of this office with Dolores Cottone of your staff on April 11, 1988. This inquiry concerned activities authorized by License No. SNM-1814, as they relate to the pending need for renewal of your license.
From this discussion, it is our understanding that no pacemakers have ever been l implanted under the authority of this license. It is also.our understanding that the only patient that was followed after being transferred to your program ;
has died and the device was explanted and returned to the manufacturer. We also understand that a letter shall be sent to this office which will either request renewal of your license or its termination.
If our understandings are incorrect, please inform us in writing. l In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2, l Title 10, Code of Federal Regulations, a copy of this letter will be placed in i the Public Document Room.
