Inquiry No.88-001 This refers to a telephone inquiry by Steven Courtemanche of this office with Dolores Cottone of your staff on April 11, 1988.
This inquiry concerned activities authorized by License No. SNM-1814, as they relate to the pending need for renewal of your license.
From this discussion, it is our understanding that no pacemakers have ever been implanted under the authority of this license.
It is also.our understanding that the only patient that was followed after being transferred to your program has died and the device was explanted and returned to the manufacturer. We also understand that a letter shall be sent to this office which will either request renewal of your license or its termination.
If our understandings are incorrect, please inform us in writing.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter will be placed in i
