FERMI INSPECTION REPORT 50-341/98012(DRP)
Dear Mr. Gibson:
On August 3,1998, the NRC completed an inspection at the Fermi 2 reactor facility. The
enclosed report presents the results of that inspection.
During the 6-week period covered by this inspection, the plant experienced a forced outage
caused by the recurrence of power oscillations due to secondary system equipment problems.
Your evaluation of the event was comprehensive, resulting in potential root cause determination
<
and initiation of corrective actions that required one high pressure turbine throttle control valve to
be closed for the remainder of the cycle. In addition, the report discusses a number of procedure
discrepancies identified in surveillance and work related procedures. The NRC is concemed with
-
. the number of procedure discrepancies identified during the inspection period. Your continued
'
attention to procedure adequacy is warranted.
Based on the results of this inspection, the NRC has determined that a potential violation of NRC
' requirements occurred. The potential violation is described in detail in the subject inspection
report. . This potential violation is of concem because it involves a repetitive failure to implement
effective corrective actions, which resulted in a violation of Technical Specification requirements.
Because your staff was in the process of determining appropriate actions to address the broader
corrective action concem at the end of the inspection period, this violation is being characterized
as a potential violation. After you have completed your assessment of those corrective actions
necessary to prevent reoccurrence, the NRC will process the violation in accordance with the
