PRELIMINARY NOTIFICATION April 5, 1977
- f PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-77-51 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.
Licensee: Kay-Ray, Incorporated, Arlington Heights, Illinois License No. 12-11184-01 Licensee Subj ect:
POSSIBLE EXCESSIVE EXPOSURE TO A RADIATION WORKER a
The licensee informed RIII (Chicago) on April 4, 1977, by telephone of the receipt of personnel monitoring results which indicated a whole body exposure of 82 rem and an extremity exposure of 115 rem for one of their employees. The licensee's initial evaluation has not been able to identify the source of this exposure which is reported to have occurred during the period from March 7 - 21, 1977.
Region III immedia.'ely dispatched an investigator and a Radiation Specialist to the licensee's facility to initiate an investigation into this matter. Their investigation is continuing.
The State of Illinois has been informed of this event. There has been no publicity so far. The results of their continuing investigation will determine whether or not the Region will issue a news release.
