The information presented is as initially received without verification or evaluation ) and is basically all that is known by IE staff on this date. Inadvertent Safety Injection i The licensee reported to the Region V (Walnut Creek, California) office that { a spurious safety injection (SI) with accompanying automatic reactor trip occurred at approximately 10:15 a.m. on March 2, 1977, and that the plant was expected to remain shut down for miscellaneous planned maintenance until ] the evening of March 3. The licensee silbsequently (on the morning of March 3) reported anomalies associated with attempts to reset the safeguards functions initiated by the spurious safety injection, and the failure ol' one of the SI pumps to start in response to the SI initiation signal. Anomalies with the safeguards initiation and reset circuitry relate to d_ ' (1) a second safeguards initiation approximately 15 minutes following the initial initiation, even though the SI circuit had been reset and " blocked;" and (2) apparent failure of four SI circuitry output (slave) relays to reset (unlatch) and allow manual recovery of automatically started SI equipment by the plant operators. j Two Region V inspectors were dispatched to the site on March 3, and will remain through the current weekend. Details of the occurrence and remedial actions by the licensee are being examined by the Region V N inspectors. Potential generic aspects of the occurrence are being co.nsidered and have. been and will continue to be communicated to IE:HQ and the NRR Project Manager.
