Dear Sir:
This letter will serve as written verification of the follow-up contact made on Mr. Harry Williams, who received a nuclear pacemaker implant in the Cardiovascular Study Unit at Hurley Medical Center in 1974.
Hurley Medical Center is currently following one patient with a nuclear pacemaker implant. The following information was obtained from the patient Tuesday, August 25,1998.
HARRY WILLIAMS-MODEL #9000A-Serial #3R00248-Implanted 04/30/74.
Currently living in Grubbs, Arkansas, Telephone Number (870)252-3428. His physician is Wm.
Robinette, M.D., Family Practice Clinic, 801 Osler Drive, Jonesboro, Arkansas, 74213, Telephone Number (870)932-2423. Mr. Williams' pacemaker has not been replaced. Mr.
Williams indicated that he is not experiencing any complications due to the pacemaker implant.
