This rofers to the letter dated March 11, 1988, from Mr. Ed Koopman of your organization, and to the telephone conversation between you and S. R. Lasuk of this office on April 4, 1988. The letter, which was addressed to Darrel Wiedeman of my staff, reported the loss of a 3M, Model 902, static eliminator (Serial No.13836) containing a nominal 10 milliceries of polonium-210. These devices are possessed under the authorization of a NRC General License pursuant to 10 CFR 31.5.
As a result of the reported loss and subsequent conversation with you, certain of your activities appear to be in violation of NRC requirements as specified in the enclosed Notice, and a written response is required.
The responses directed by this letter and the accompanying Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980,.PL 96-511.
We will gladly discuss any questions you have concerning this inspection.
