SPECIAL REPORT ~ 87 -12 RESULTS OF THIRD STEAM GENERATOR TUBE INSERVICE INSPECTION This report is submitted in accordance with Technical Specification (T/S) 4.4.5.5.b which states:
"The complete results of the steam. generator tube inservice inspection shall be submi ted to the Commission in a Special Report. pursuant to Specification 6.9.2 within 12 months following the completion of the inspection.
This Special Report shall include:
(1) Number and extent of tubes inspected, (2) Location and percent of wall-thickness penetration for each indication of an imperfection, and (3)
Identification of tubes plugged."
From 9/15/87 to 10/2/87 with the plant in Mode 6 - Pefueling for the second refueling outage, the third plant Steam Generator (S/G) Eddy Current Inservice Inspection was performed.
S/G's 'A' and 'D' were inspected. The inspection was performed by Westinghouse Electric Corporation and their formal results are detailed in the Field Service Report (FSR) which is submitted with this Special Report.
Although the Westinghouse FSR provides details on the inspection, the results are summarized below and are numbered to correspond with the information requested by T/S 4.4.5.5.b:
(1) Number / Extent of Inspected Tubec (a) Steam Generator 'A':
A total of 3507 tubes were tested. Three thousand one hundred forty-eight (3148) were tested full length, 204 to the 1st support Hot Leg side, and 155 tested through the U-bend.
Three (3) tubes were plugged as a result of eddy current in9.ications.
One (1) of these tubes was an Anti-Vibration Bar indication, one tube as a result of a dent signal at the top of the tubesheet, and one was a support plate indication, (b)
Steam Generator 'D':
A total of 3508 tubes were tested.
Three thousand one hundred forty-four (3144) were tested full length, 209 to the 1st support Hot Leg side and 155 tested through the U-bend.
Five (5) tubes were plugged as a result of eddy current indications.
Four (4) of these tubes were Anti-Vibration Bar indications.
