Subject: NRC Region I Inspection Report No. 50-223/88-01

This refers to the routine safety inspection conducted by Messrs. J. Kaucher

and A. Cerne of this office on March 14-16 and 22,1988 of activities author-

ized by NRC License No. R-125 and to the discussions of our findings held by

.

Messrs. Kaucher and Cerne with you and other members of your staff at the

conclusion of the inspection, and to a subsequent telephone discussion between

Mr. D. Haverkamp of this office and you on April 7, 1988.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. Within these areas, the

inspection consisted of selective examinations of procedures and representative

records, interviews with personnel, and observations by the inspectors.

During the conduct of the inspection, several areas of concern were identified,

namely; three apparent violations concerning (1) failures to meet Technical

Specification required activities, (2) degradation in the performance of the

Reactor Safety Subcommittee and (3) inadequate review and approval of proced-

ures as required by Technical Specifications. Another issue of concern which

will be addressed separately by the--NRC involves the fitness for duty of an

individual at the reactor facility.

In_ light of the serious nature of these concerns, we have scheduled an enforce-

'- ment conference with you to be held at Lowell, Massachusetts, on April 29, 1988.

We will forward an appropriate notification of enforcement action after the

enforcement conference.

During the enforcement conference, please be prepared to address the following:

--

the safety significance of the apparent violations

-- the root cause evaluation for each apparent violation

--

those factors that worsened or mitigated the apparent violations

--

the broader implications of the apparent violations

l

--

those actions planned or taken to prevent their recurrence, and

l

--

your views regarding the additional concerns discussed in sections 5,

6 and 7 of the enclosed report.
