During an NRC review cf your Form NRC-241 dated March 14, 1988, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:
10 CFR 150.20(b)(1) requires that each licensee wishing to engage in licensed activity in a Non-Agreement State who holds a specific license in an Agreement State, shall, at least 3 days before engaging in each such activity, file 4 copies of Form NRC-241, and 4 copies of its Agreement State specific license with the appropriate NRC Regional Office.
Contrary to the above, the Form NRC-241 that was submitted was dated March 14, 1988, which is 5 days after activity was begun in a Non-Agreement State and 3 days after work in the Non-Agreement State was 1
finished.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Professional Service Industries, Inc. is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for the violation:
(1) tne reason for the violation if
- admitted, the corrective steps which have been taken and the results
- achieved, the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance vill be achieved.
Where good cause is shown, consideration will be given to extending the response time, i
