COMANCilE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET NOS, 50-445 AND 50-446 TVBE STEEL MANUFACTURING DEFECTS SDAR: CP-88-28 (INTERIM REPORT)

Gentlemen:

On March'14, 1988, we verbally notified your office of a deficiency involving .

cracks found in several sections of tube steel utilized in the site fabrication of lipe supports. Specifically, during the assembly of a pipe  ;

support, a crac( on the internal radius of a piece of tube steel was  ;

identified. Further investigation has identified similar defects on tube -

steel supplied by the same manufacturer and received on the same Receiving inspection Report (RlR).

In addition, some of the tube steel sections under evaluation appear to be undersized.  ;

in order to assess these deficiencies, a metallurgical investigation has been  :

performed to determine if the steel plate used to fabricate tube steel was '

defective. Preliminary results indicate the base material meets the chemical and material properties of the specification and suggests the deficiencies

  • occurred during the manufacturing process, t

We are continuing our evaluation of the manufacturing process at the supplier facility. Additionally, the supplier is examining records in order to establish unusual conditions which might have caused the cracks on the day of manufacture.

A hold has been placed on the remaining (uninstalled) tube steel received i under the same purchase order. Corrective action to be taken will depend upon the final results of our investigation. This activity will be completed prior to Unit I and 2 fuel load.
