Dear Mr. Kramer:
In order to determine whether to conduct further inspection of your activities at NRC-licensed facilities, the NRC requires that you provide further informa-tion. Therefore, pursuant to Section 161c. of the Atomic Energy Act of 1954, as amended, Section 206 of the Energy Reorganization Act of 1974, and the provisions of 10 CFR Part 21 the NRC requires that you respond to the following questions:
1.
Iaentify each and every instance where you have provided services to the nuclear industry since 1985.
For each instance, state the entity for which you performed the service, the dates and location of the service, and the nature of the service which you provioed.
If you have provided none, so state.
2.
Have you ever provided services to the nuclear industry under any name other than Power Inspection, Incorporated?
If so, state under what name you have provided such services.
For each such instance, state the entity for which you provided such services, the dates and location of the service, and the nature of the service which you provided.
3.
Have you ever provided services to the nuclear industry from any office or facility other than your facility at Wexford, Pennsylycnia?
If so, state the name and location of each such office or facility, the-entities for whom you performed services, the dates and location of the service, and the nature of the service which you provided.
4.
Have you been affiliated with other businesses which have provided services to the nuclear industry? If so, state the name and location of each business with which you have been affiliated, the dates of such affiliation, and each entity for which you provided services as part of such affiliation, the dates and location of the service, and the nature of the service which you provioed.
You are hereby required to submit to this office by certified mail within 30 days of the date of this letter your response to the questions set forth above. This response should be made in writing, and sho' d be signed under oath or affirmation.
