This refers to the special safety inspection conducted by Mr. Robert E. Burgin of this office on June 11, 1988, and to the discussion of our findings with you at the conclusion of the inspection.
This special inspection wa's the result of NRC Region III (Chicago) being notified through Region II (Atlanta) that potentially contaminated cartons / boxes of various materials sterilized at the Radiation Sterilizer, Inc. (RSI) facility in Decatur, Georgia had been shipped to manufacturing and distribution facilities in Region III. The purpose of this site visit to Amway Corporation in Ada, Michigan was to perform surveys of boxes and cartons received from RSI in May 1988 (RSI had identified' cesium-137 contamination on irradiated packages still in their Decatur, Georgia facility as a result of a leaking cesium-137 i
WESF capsule and the potential for contamination on packages shipped from RSI subsequent to April 29, 1988).
The enclosed inspection report provides background information and specific survey results.
