It is on the proper notification phone number that we request clarification.

In our SNM license application we incorporated our protocols for the hcndling of the source material upon the death of a pacemaker patieni.

The phone number listed in the protocol vse (301) 427-4112 ottention Ms. Pat Vacca.

However, upon the death of a nuclear p cemaker patient last month we discovered that this number was no longer accurate for the reporting procedure.

Another number was finally obtained for the reporting of this event (301) 492-0615.

We cre unclear as to whether to contact the USNRC Region I as the license states in condition number 12 or the phone number listed cbove incorporated into our license application and protocols.

Your timely written clarification on this matter is needed in order to comply with license committments and avoid any future reporting difficulties.
