t NOTICE OF VIOLATION Sacramento Municipal Utility District Docket No. 50-312 P. O. Box 15830 License No. DPR-54 Curing an NRC inspection conducted on December 5.-1987 through February 3,1988 a violation of NRC requirements was identified, in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2 Appendix C, the violation is listed below:

10 CFR 50 Appendix B, Criterion V, states, in part:

"Activities affecting quality shall be prescribed by documented instructions, procedures...and shall be accomplished in accordance with these instructions, procedures..."

Quality Assurance Procedure QAP-17 Revision 7, dated November 25, 1987, states, in part:

"5.2.9 Closure _o_f NCRs 1

1 5.2.9.1 The Quality Engineer shall sign and date Block VIII to show evidence of completed rework, repair, released calculations, ECN or DCN (Transmittal No.) released for work, or Retest of System / Unit Complete. The Quality l

Engineer's signature in Block Vill means that all required l

actions in Block II have been completed. This verification rray require Work Request review, procedure i

review, drawing review, etc. Also, if applicable, the 10 CFR 50.59 Determination Form must be completed prior to NCR closure, h0TE:

If an ECN/DCN has been issued in response to a NCR cisposition, the ECN/DCN must be closed by site docun.ent control prict to NCR closure.

5.2.9.2 The QE shall perform a final review of the NCR to ensure j

Contrary to the above:

On July 2, 1987, NCR 56053 was signed and closed by a Quality Engirter without corrective actions in Block II of the NCR for wire IM1B180B being perforned.

On December 8, 1987, NCR 57295 was signed and closed by a Quality Engineer when the corrective actions proposed were irecrocrated into ECN R1590 without prior closure of the ECN by Site Document Control.

On December 11, 1987 NCR 57272 was signed and closed by a Quality Engineer when the disposition in Block II of the NCR which required a hydrotest had not been perfomed.

l This is a Severity Level IV violation (Supplement 1).

This reply should be clearly marked as a "Reply to ? Notice of j

(1) the reason for the i

i violation if admitted, (2) the corrective steps that have been taken and the results achieved. (3) the corrective steps that will be taken to avoid further i

i violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, e

suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the response time for good
