Trojcn NuclcCr Pltnt Document Control De;k Dock t 50-344 July 14, 1988 Licenso NPF-1 Attachment RESPONSE TO A NOTICE OF VIOLATION Violation Part 20.201(b) to Title 10 of the Code of Federal Regulations (10 CFR 20.201(b)) requires that each licenseo make such surveys as (1) are necessary to comply with regulations in 10 CFR 20, and (2) are reasonablo under the circumstances to evaluate the extent of radiation hazards that may be present. As defined in 10 CFR 20.201(a), "survey" means an evaluation of the radiation hazards incident to the production, uso, releaso, disposal, or presence of radioactivo materials or other sources of radiation under a specific sot of conditions.
Contrary to the above, on May 8,1988, no survey was made to determino the direct dose to a reactor operator's hand from handling highly con-taminated plastic tubing.
The doso from residual contamination found on his hand was appropriately evaluated.
This is a Severity Level IV Violation (Supplement IV).
Responso Portland General Electric Company (PCE) acknowledges the violation.

Reason for Violation:
The cause of the violation was personnel error.
Though the sourco of the hand contamination was ioentified as being contamit.ation on the drain hose, the direct exposure potential from a hot particle en the hose was not recognized and identified on the Radiological Event Report and Contamination Report for the incident.

Correctivo Action Taken and Results Achieved:
An evaluation of the skin exposuro from both contact with the hot l
particle on the hose and the residual contamination was completed.
Calculations indicated that the skin dose was less than the limits specified in 10 CFR 20.

Ccerective Action to Avoid Further Violations:
The appropriate Radiation Protection Branch personnel have been cau-tioned to be more aware of the need to evaluate exposures when hot particles are found in the Plant that may have been handled by personnel.

Date When Full Compliance Will be Achieved:
Full compliance has been achieved since the dose to the operator's hand from the contaminated hose has been properly evaluated.
