2 1.0 SCOPE 1.1
-This Quality Assurance Program responds to the requirements of Subpart H of 10 CFR Part 71 and is applicable to United States Testing Company, Inc.
Unitech. Services Group radioactive sources which are included in the scope of 10 CFR 71.
2.0 ORGANIZATION V
2.1 Design and fabrication of radioactive material shipping packages shall not be conducted under this Quality Ass-urance Program.-
2.2 The final responsibility for the Quality Assurance Program for Part 71 Requirements rests with the United States Testing Company Inc., Unitech Services Group.
2.2.1 The Quality Assurance Program is implemented using the or-ganization chart seen in Exhibit A.

3.0 RESPONSIBILITIES 1
3.1 The Director, Office of Radiation Safety and Quality Ass-urance is responsible for overall administration of the program, training and certification, document control 1
and auditing.
3.2 The Regional Radiation Safety Officers are responsible for assisting the Radiation Safety Officers that are in their perspective regions, on matters relating to this program.
J They report directly to the Director, Office of Radiation i
Safety and Quality Assurance for their specific tasks and administrative guidance.
3.3 Radiation Safety Officers are assigned for each particular work location.
They are responsible to the Regional Radia-i tion Safety Officer for the administration of the program, training and certification, document control and overall surveillance of those activities pertaining to the assigned 4
work locations.

j 4.0-QUALITY ASSURANCE PROGRAM 4.1 The management of United States Testing Company, Inc.,
Unitech Services Group establishes and implements this i
Quality Assurance Program.
Training for all QA functions,
~
i prior to engagement in these functions, is required accord-l ing to w'titten procedures.
QA Program revisions will be made according to written procedures with management approval.-
The QA Program will insure that all defined QC procedures, engineering procedures, and specific provisions of the pack-age design approval are satisfied.
The QA Program will em-phasize control of the characteristics of the package which are }
critical to safety.
4.2 The Director, Office of Radiation Safety and Quality Assurance shall assure that all radioactive material i
shipping packages are designed and manufactured under i
a Quality Assurance Program approved by the Nucicar Regulatory Commission for all packages designed or r
i fabricated after January 1, 1979.
This requirement can be satisfied by receiving a certification to this i
effect from the manufacturer or supplier of the package.
5.0 DOCUMENT CONTROL 4
5.1 All documents related to a specific shipping package will be controlled through the use of written procedures.

document changes will be performed according to written procedures approved by management.

6.2 Radiography personnel shall perform the critical handling, storage and shipping operations.
7.0 INSPECTION, TEST AND OPERATING STATUS 7.1 Inspection, test and operating status of packages for cer-tain special form radioactive material will be indicated and controlled by written procedures.
Status will be in-dicated by tag, label, marking or log entry.
Status of con-forming parts of packages will be positively maintained by written procedures.
7.2 Radiography personnel shall perform the regulatory required inspections and tests in accordance with written procedures.
The Radiation Safety Officer shall ensure that these func-tions are performed.
8.0 QUALITY ASSURANCH RECORDS 8.1 Records of package approvals (including references and drawings), inspections, tests, operating logs, audit re-l suits, personnel training and qualifications and records of shipments will be maintained.
Descriptions of equip-ment and written procedures will also be maintained.
8.2 These records will be maintained in accordance with writt-en procedures.
The records will be identifiable and re-trievable.
A list of these records, with their storage locations, will be maintained by the Director, Office of Radiation Safety and Quality Assurance or his designee.
9.0 AUDITS 9.1 Established schedules of audits of the Quality Assurance Program will be performed using written checklists.
Re-sults of audits will be maintained and reported to manage-ment.
Audit reports will be evaluated and deficient areas corrected.
The audits will be dependent on the safety sig-nificance of the activity being audited, but each activity will be audited at least once per year.
Audit reports will be maintained as part of the quality assurance records.
Members of the audit team shall have no responsibilty in the activity being audited.
