tion to those otherwise required, of the Shipper's Export Dectoration covering each export and mark one of such copies for transmittol to the U.S. Nuclear Regu-lotory Commission, Washington, D.C. 20555. The following declaration should oc-company or be placed on the Shipper's Export Dec.lorotions for such exports: ,
"This shipment is being mode pursvont to specific heense number (specific license number) filed at (location of Customs office where license is filed),
on (date license was filed). Th s license expires on (expiration date of license), and the unshipped bolonce remoining on this hcense is sufficient to cover the shipment described on this declaration.
Condition 2 - Exports outhorized in any country or destinction, except Country Groups Q. 5, W, X, Y, and Z in Port 3/0, Supplement No.1. of the Comprehens ve Export Schedule of the U S. Department of Commerce.
Condition 3 - This beense covers only the nuclear content of the rnatericl Condition 4 - The meterial to be exported under this hcense shall be sh pped in occordance .wh the physical protection teovirements for special nuclear mo'erial in 10 CFR 73.
Condition 5 - Special nuclear meteriol authorized for esport under th 5 1 cense shall not be transported outside the United States in possenger cariying circeof t in shipments exceeding (1) 20 groms or 20 curies. whichever is less, of plutonium oi vronium 233, or (2) 350 grams of vronium 235 Condition 6- This hcen;e authorizes esport only and does not outhonte the rece pt. phys col possession, or use of the nuclear materiol.
Condition 7 - The hcensee shall complete and submit on NRC Form 7.51 for each sh.pment cf source material exported under this hcense.
Condition 8 - The hcensee shall advise the NRC in the event there is ony change in the designo-tion of the company who will pockoge tFe nuclear material to be exported under this license, or any change in the locotion of the packaging operation, at least three weeks prior to the scheduled dote of export.
Condition 9 - The material to be exported under this license shall be protected in transit, while within U.S. jurisdiction, in accordance with the requirements of 10 CFR Part 73 and the licensees approved security plan.
