Revision No. O to Certificate of Compliance 9122 was issued on November 6, 1978 in response to your June 27, 1978 application.
A $200 application fee and $1200 approval fee were paid for the request.
In accordance with Footnote 4 of the enclosed 10 CFR 170, we have reviewed the manpower expenditures required for the review of the application, and the actual review cost is $3990.
