ATTACE CNT I Respense to Actendix A - [ctice of Violaticn Description of Infraction 10 CFR 50, Appendix 3, Criterien III states, in part, dat "deasures shall be established to assure cat applicable regulatory requirements

...are correctly translated into specification, drawings, procedures and instractions. These measures su' d"alude provisions to assure that apprcpriate quality standards are specified and included in design documents and that deviations frc= such standards are centrolled..."

j Paragraph D.7 3 of the Quality Assurance Program documented in the PSAR states, in part, that "... itens included in the 3echtel Engineerirg design centrol (are)... deter =ine that de required design bases, ?SAR cc==it=ents and o der regulatory requirements i

are incorporated prcperly into the design drawings..."

Paragraphs C.2.6 and 5 2 5 2.1, of the PSAR, specify tne A=erican Welding Society (AWS) Code D1.1 as a quality standard for Class 1 seis=ic structures.

Contrary to de above, as of May 16, 1978, the =easures prev!.ded did not assure ca unacceptable deviations frc= qualit/ stard " *r Class 1 seis=:.c structures were centrolled.

A.

Structural steel was anstalled in,,he centai.=ent, a Class 1 seismic structure, per Sechtel draw:.ng No. C-875, Revision 6, usir4 weldi 4 procedures for prequalified fillet weld jcints. The weld joints do not meet ce criteria in Section 2 7 of de #45 D1.1 cede for pre-qualified fillet weld joints.

3.

Stractural steel was installed in the containment, a Class 1 seismic structure, per Eechtel draw:.rg No. C-575, Rev sien 6, using bars to fill large weld jcint gaps.

'"he application of te bars does not test de criter:.a in Section 2.6 cf the f4S D1.1 Ocde f6 of l

fillers.

l Response to Item A l

The weld joints shown in Details LA and 9 cn Dws 8031-C-375,Rev. 6, are censidered prequalified welds as either fillet welds of f45 D1.1*

Fig. 2 7 1 types or partial penetratien pceve welds of de BC-P2 cr BTC-Ph types as applicable under f45 D1.1 Fig. 2.10.1.

The approved weldir4 precedure used, Bechtel welding procedure F1-A-Lh (Struc'; ural) is applicable for both fillet welds and partial penetration g oeve welds.

Calculations indicate that the partial penetration and/cr fillet welds shown in Details LA and 9 of Dws. 3031-C-875 are capable of

{

effectively acccc=odating te design loads.

To clarify requirements, Dwg. 3031-C-875 has been revised to show te vertical weld between te transi*,ien plate and -2.e locse plate as a fillet weld when te included argle between de base =aterials is Feater than 60' and as a partial pe:;e: ration pocve weld when the "A

included argle is betueen h5 Rescense to Item 3 Detail 9 of Dws. 3031-C-875, Rev. 6, was developed in order to resolve an assembly problem as provided by MTS D1.,1 para. 2.h.1.2.

Section 3 3 cf AWS indicates dat when dere is an assembly prcblem corrections approved by de ergineer are to be provided. As required by El3, Detail 9 was developed and approved by Bechtel - Project Ergineerir4 as a corrective ceasure.

Detail 9 is in eccpliance with Section 2.h in dat all filler plates as fabricated by Bechtel at de site are inch er ticker. To be sure cat there is,no =isunderstandir4, Dws. 3C31-C-875 has been revised to show a ; inch -4 d u: tickness for te filler plates.

Detail 9 clearly shows at de filler plate =st be cf sufficient sice so tat w dc "t w" is centinuous trcugh the filler plate. No fur te" 'd-d tatiens en plate dimensions are required per ergineeri 4 design.
