Based on inspections conducted on September 13-15, 20, 21, 27-29, and October 4 and 5, 1978, it appears that certain of your activ-ities are in noncompliance with NRC requirements as noted below.
This item of noncompliance is considered to be an infraction.
Contrary to Technical Specification 6.2. A.6 a written and approved procedure was not adhered to during the periormance of preventative maintenance on a safety related component.
