I Appendix A NOTICE OF VIOLATION Riverside Hospital License No. 34-13234-01 Based on the inspection conducted on August 11, 1978, it appears j that certain of your activities were in noncompliance with NRC requirements, as noted below. Items 1 and 2 are infractions, and items 3 and 4 are deficiencies.

1. 10 CFR 20.101(a) limits the whole body exposure of an individual in a restricted area to one and one quarter rems per calendar i quarter, except as provided by 10 CFR 20.101(b). Paragraph (b) allows a whole body exposure of three rems per calendar quarter provided certain specified conditions are met.

Contrary to this requirement, an individual working in your restricted area received a whole body radiation dose of 1.380 rem during the fourth calendar quarter of 1975 and conditions of paragraph (b) were not met.

2. 10 CFR 20.405(a) requires that you submit within 30 days, a report to the Commission concerning each exposure to radi-ation in excess of any applicable limit in Part 20.

Contrary to this requirement, as of the inspection date, you failed to report to the Commission an exposure in excess of a j limit in Part 20, namely, a whole body radiation dose of 1.380 rem during the fourth calendar quarter of 1975.

3. 10 CFR 19.13(a) states that when a licensee is required by 10 CFR 20.405 to report to the Commission an exposure of an individual to radiation the licensee shall also provide that individual a report on his exposure data.

Contrary to this requirement, as of the inspection date, you failed to report a whole body radiation dose of 1.380 rem during the fourth calendar quarter of 1975 to the exposed individual.

4. 10 CFR 20.401(a) requires that you maintain records showing the radiation exposures on Form NRC-5, in accordance with the instructions contained in that form or on a clear and legible record containing all the information required by Form ERC-5.

Contrary to this requirement, on the day of the inspection, your exposure records did not contain the required information.

Specifically, they did not list the social security number or birthdate for the personnel for whom monitoring is required.
