are an Industrial Radiographic Inspection cornpany licensed for Iridium 192 and Cobalt 60 gamma ray sources in the state of North Carolina. We trans? A these sources in approved exposure devices in company vehicles through out the state. As yet we have not gone to another state. We want to be able to perform radiography on the military instillations here in the state and this requires a license of reciprocity and this procedure approved in acmrdance with 10 CFR part 71. Please review and approve this procedure. If you have any questions please call me at (919) 355-9353. A copy of our North Carolina Radioactive Materials License (Amendment 5)is enclosed.

A.

To be used in a Source Production & Equipment Co., Inc. Model Spec 2-T exposure device for industrial radiography and in Source Production & Equipment Co.,

Inc. Model Spec C-1 storage and source changer.

To be used in a Source Production & Equipment Co.,Inc., Model SPEC-150 radiographic i

exposure device for industrial radiography. Also, to be used in a Source Production &

Equipment Co., Inc. Model C-1 source exchanger for source exchange and storage.

CONDITIONS 10A.

Radioactive material may be stored and used at the licensee's facility located on Highway 11 South, Winterville, NC 28590.

B.

Radioactive material may be used at temporary jobsites of the licensee anywhere in North Carolina. The North Carolina Radiation Protection Section shall be notified of the location (s) of such use no later than three (3) days prior to the first day of use.

This condition does not prohibit use in the United States where the United States Nuclear Regulatory Commission maintains jurisdiction for regulating the use of radioactive material, nor in Agr ement States under reciprocity procedures which may be established by the Nuclear Regulatory Commission and/or the respective Agreement States.

If for a specific case the three-day period would impose an undue hardship on the licensee, he may upon application to the Agency obtain permission to proceed sooner.

Protection Against Radiation," and 15A NCAC 11.1000, " Notices, Instructions, Reports and Inspections."

(The North Carolina Regulations for Protection Against Radiation are l

contained in 15A NCAC 11.)

12A. Radioactive material shall be used by Thomas H. Hartley or persons who have successfully completed the training program described in The Roberts Company Radiation Protection l

Procedures Manual. The licensee shall keep records of each person's training and qualifications that is authorized under this condition.

B. The Radiation Safety Officer for the activities authorized by this license is Thomas H.

Hartley.

l 13.

Pursuant to 15A NCAC 11.0507, the licensee is authorized to perform tests for leakage or contamination of the sealed sources authorized by this license, or tests may be performed by other persons specifically authorized by this Agency to perform such services.

14.

Notwithstanding the periodic leak test required by 15A NCAC 11.0507, such requirement i

does not apply to radiography sources that are stored and not being used. The sources I

exempted from this test shall be tested for leakage prior to any use or transfer to another person unless they have been leak tested within six months prior to the date of 1

use or transfer.

I 15.

The licensee is authorized to receive, possess and use sealed sources of Iridium 192 or Cobalt 60 where the radioactivity exceeds the maximum amount of radioactivity specified in Item 8 of this license provided:

A.

Such possession does not exceed the quantity per source specified in Item 8 by more than 20% for Iridium 192 or 10% for Cobalt 60; i

B.

Records of the licensee show that no more than the maximum amount of radioactivity per source specified in Item 8 of the license was ordered from the supplier or transferor of the radioactive material; and C.

The levels of radiation for radiographic exposure devices and storage containers do not exceed those specified in 15A NCAC 11.0503.

16.

The licensee is authorized to possess, use and transfer up to 1000 pounds of Uranium contained as shielding material in the radiography exposure devices authorized by this license.

The licensee shall conduct a program for inspection and maintenance of radiography 17 exposure devices and storage containers to assure proper functioning of components important to safety.

The inspection and maintenance program shall be conducted at intervals not to exceed three (3) months. Records of the results shall be maintained for inspection by the Agency.

18.

The licensee shall not transfer possession and/or control of materials or products containing radioactive material as a contaminant except:

A.

By transfer of waste to an authorized recipient; B.

By transfer to a specifically licensed recipient; or C.

As provided otherwise by specific condition of this license pursuant to the requirements of 15 NCAC 11.1628.

19.

The licensee may transport licensed material or deliver licensed material to a carrier for transport, in accordance with the provisions of Section 71.5, Title 10, Code of Federal Regulations, Part 71, " Packaging of Radioactive Material For Transport."

The licensee shall conduct a physical inventory every six (6) months to account for all j

sealed sources received and possessed under the license. The records of the inventories shall be maintained for two (2) years from the date of the inventory for inspection by the Agency and shall include the quantities and kinds of radioactive material, location of sealed sources, and the date of the inventory.

l 21.

Except as specifically provided otherwise by this license, the licensee shall possess and l

l use radioactive material described in Items 6, 7 and 8 of this license in accordance with statements, representations and procedures contained in:
