Dear Dr. Murley:
Haddam Neck Plant Steam Generator Tube Plugging Report Plant Technical Specifications require that the number of tubes plugged in each steam generator be reported to the commission within fifteen days following inservice inspection of steam generator tubes.
A total of 126 tubes were plugged.
119 tubes were plugged due to flaws within the top one inch of hard roll.
Two tubes were plugged due to leakage. Two tubes were accidently plugged due to plugging vendor mistakes. One tube was plugged which was missed during our previous winter 1986 refueling outage. One tube was plugged due to suspicion of leakage (boric acid buildup around tube end). Finally, one tube was plugged due to an indication in its roll transition zone, a flav which was identified as the cause for leakage of one of the leakers mentioned abo.e.
The number of tubes plugged in each steam generator is shown below:
