RCIC Inoperable While HPCI Is Out of Service Date of Event:

February 7, 1984 Plant:

Fitzpatrick EVENT DESCRIPTION Sequence During normal full-power operation the HPCI system was intention-ally removed from service to permit maintenance and,:.modification.ý Before its removal, completion of surveillance required by Technical Specifications had demonstrated the ADS, RCIC, LPCI, and both CS systems to be operable.

While the HPCI system was out of service, the RCIC system was made inoperable by a personnel error.

Corrective Action Corrective action to reduce the probability of recurrence includes disciplinary action for the persons involved, implementation of a pro-.,

gram to verify that employees are knowledgeable of the equipment they work on, continuation of plant-specific BWR technology/training, and labeling of certain plant equipment.
