The
- anclosed report gerents the results of that inspection.
During tbs 5-week period covered by this inspecthn perin<!, your conduct of activities at the plant $were generally charactenzed by safe phot operation. Conservetive decision making was demonstrated by preparing f0 and inserting a manual reactor scram when faced with a potentia! loss of reactor pressure control due to a turbine controi valve feilura. The cause of the turbine control valve taiiure was due to the failure of the push rod spdng housing coupling bolts c'oe to previous improper maintenance. Your disposition of vendor information : elated to turbine control vaive maintenance did not prevent iho TGV failure and resulting plant transient.
