with Applicable Material Specifications". This bulletin requested that l

licensees 1) review their receipt inspection recuirements and internal controls for fasteners and 2) independently determine, through testing, whether fasteners (studs, bolts, cap screws and nuts) in stores at their facilities meet reauired mechanical and chemical specification requirements.

Attachment a to AECM-88/2-0002 1.b) Internal Controls for Storage and Use of Fasteners 1)

Storage Controls:

a.

Access to the warehouse is limited to authorized personnel, b.

Fasteners are controlled during storage by segregation into containers / bins by type, size, and clessification.

2)

Issue Controls:

a.

Authorized personnel issue fasteners only after an approved Material Withdrawal Request is received identifying the material type, size and classification.

3)

Fasteners are visually inspected at time of installation to verify traceability (heat code),

correct material (markings), size and length to meet the design requirements.

Select a minimum sample of ten (10) non-safety)related fasteners Iten 2)

(studs, bolts, and/or cap screws), and ten (10 safety-related fasteners (studs, bolts, and/or cap screws) from current, in use, steck. The sample is to be obtained by the licensee with the participation of an NRC inspector.

Fasteners procured to meet the following chemical and mechanical properties are of interest: A-193 gredes B7, 88, and B16; SAE J429 grades 5 and 8; A a49; A-325 Types t

1, 2 or 31 A-354 grades BB, BC BD; A-490; A-320 LTM; A-307; A-563; or equivalent.

  • Pesponse: 2.0) Twenty-one samples of safety related and non-safety related fasteners were : selected by SERI with the participation of the i;RC resident irispector.

Chemical testing shall be performed on all samples. Mechanical l

testing shall be performed on each safety-related fastener.

Hardness testing shall be perforned on each nut and non-safety-related fastener. All testing shall be performed by a laboratory which the licensee has qualified for this type of testing and appears an the i

licensee's approved vendor list.

. to AECM-88/2-0002 accordance with the requirements of the fastener's specification, grade, and class, and the test shall evaluate the ultimate tensile strength, hardness and chemical properties as required by the fastener's specification, grade, and class.

Each sample shall be tagged with the sample's ID number.

Item 5)

The results of all tests, together with supporting information, are to be rept;rted to the NRC utilizing the format shown in Attachments 1 and 2 of this bulletin.

Include the names and addresses of suppliers and manufacturers of safety-related fasteners and, to the extent possible, of non-safety-related fasteners.

For any fastener found out of specification, provide an evaluation of the safety significance including consideration of the most limiting application.

Response: 5.0) The selected fasteners were shipped to Wyle Laboratories (Huntsville, Alabama) for testing in accordance with the material specification for each fastener and the testing requirements stated in NRC Compliance Bulletin 87-02.

The testing was performed by Wyle Labo*atories and the test results were documented in Wyle Laboratories' Report No. 17908-01 "Physical and Chemical Testing of Various Fasteners for Use in Grand Gulf Nuclear Power Station", dated January 20, 1988. Wyle Laboratories evaluated the test results and, with the exception cf two samples described below, found all of the fasteners to be in accordance with the applicable material specifications.

The first sample found to be out of specification was GGNS-U2-Al which consisted of two (2) non-safety related nuts (both were out of specification). The second sample found to be out of specification was GGNS-U2-10 which consisted of one (1) safety related bolt for structural applications.

SERI's Nuclear Plant Engineering performed en independent evaluation of the test results and arrived at the same conclusions.

The evaluation of the safety significance of the two samples which were found to be out of specification is provided in Attachment 2.

Based on the results of the testing and review of current procedures, describe any further actions being taken to assure that fasteners used in the plant meet the requisits specifications and requirements and that the operability of safety-related plant components is not affected.

Response: 6.0) Based on the review performed in Item 1 above, the acceptable test results and the evaluation discussed in Attachment 2, no further actions are required.

i EVALUATION OF THE SAFETY SIGNIFICANCE OF BOLTING MATERIALS FOUND OUT OF SPECIFICATION 4

Structural Evaluation: The only specification requirement which was not met was the 1

maximuri tensile load requirement.

The actual tensile load to failure for the bolt was 86,000 lbf which exceeded the specification upper limit by 7.450 i

lbf. This bolt type is only used in structural applications where the bolted connections are designed as friction connections.

In friction connections the bolt is not loaded in shear but only in tension. The bolt tension is I

established by tnrouing the bolt / nut combination to c pre-determined valve.

The design of nuclear related structures is based upon static loading i

conditions. Fatigue loading, as would be seen in commercial buildings, is nel 4

a design facter. Therefore once the bolt / nut combination has been j

pre-stressed, the variations in live loading are not sufficient to cause a j

change in the bolt stress. The bolt..as-tested, exhibited a higher than specification ultimate tensile strength which by itself would not affect the performance of the bolt in its intended application. All other characteristics 1

of the bolt were acceptable. Therefore, when considering_the loading of the i

bolt and the operating environment in which the bolt is used, there is no l

safety sicnificance to this deviation from the material specification.

j Evaluation: Thes* non-safety related nuts are used in structural applications i

only. Using the bolt /out loading consideratiens discussed above, the load on the nut is a shear loading which would tend to strip the threads of the nut.

The nuts in question satisfactorily passed the proof load test which showed that the nuts would accept the design loads without stripping or rupture.

Therefore, the nuts would have satisfactorily performed their intended i

function. Based upon the satisfactory proof load test and the fact that the nuts are intended for non-safety related applications only, there is no safety significance to this deviation from the material specification.
