NRC INSPECTION REPORT 50-482/94-17

.This refers to the routine safety inspection conducted by

,

Dr. D. Blair Spitzberg, and Messrs. Steve McCrory and Art McQueen during the

period December 5-8, 1994.

The inspection included a review of activities

,

authorized for your Wolf Creek Generating Station.

At-the conclusion of the

inspection, the findings were discussed with those members of'your staff

identified in the enclosed report.

Areas examined during.the inspection are identified in the report and include

a review of the operational status of the Emergency Preparedness Program.

Within these areas, the inspection consisted of selective examination of

procedures and representative records, interviews with personnel, and

observations by the' inspectors.

The inspection findings are documented in the

enclosed inspection report.

One weakness was identified during the inspection and is described in Section

5.1.3 of the enclosed report. The weakness concerned failure of an operating

crew to inform plant personnel of necessary protective measures following

simulated Site Area and General Emergency classifications.

Weaknesses according to 10 CFR Part 50, Appendix E.IV.F.5, are inspection

findings that need to be corrected by the licensee.

You are requested to

provide this office with a description of your corrective measures and your

schedule for completing these actions within 30 days after receipt ci this

letter.

,

In addition to the weakness, we have concluded based on the results of the

inspection, that a negative trend exists in operations training regarding

applications of the principles of a systems approach to training.

Findings

supporting this conclusion are described in Sections 5.1.7 and 9.1 of the

enclosed report.

Operations personnel errors observed during this, and other

recent inspections point to a common casual factor.

We detect a laxness on

the part of the operations training organization with regard to their

evaluation of, and response to observed performance of operations staff and

operations shift personnel in a wide scope of operational contexts.

An

indicator of this finding was the lack of effectiveness of certain corrective

actions committed to in response to a previous violation as demonstrated

during walkthroughs with operating crews. The previous violation concerned a

failure to correct a weakness in the area of emergency classification. We are

concerned that the corrective action, which related to the role of the shift

engineer in assisting shift supervisors classify emergency conditions, was not

effectively demonstrated.

as well as all safety-related operations activities.

observations related to your systems approach to operations training in

general and the closecut of this violation.
