Response to Notice of Violation - Inspection Report 50-309/87-27 Gentlemen:

Maine Yankee's response to the Notice of Violation transmitted in Reference (b) is provided below.

For completeness, we have restated the violation as it appeared in the Inspection Report.

NOTICE OF VIOLATION 10 CFR 50, Appendix B, Criterion 6 requires in part that measures shall be established to control the issuance of documents, such as instructions, procedures and drawings, including changes thereto, which prescribe all activities affecting quality.

These measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel and are distributed to and used at the location where the prescribed activity is performed.

Contrary to the above, on December 7, 1987, electrical maintenance technicians were using a diagram included in Repair Order #7277 that was not reviewed for adequacy by comparison to the latest revision of the set of controlled prints prior to being distributed and used to perform work on the motor operator to a high pressure safety injection valve, HSI-H-54.

The above instance of failure to adequately ensure the accuracy of a diagram prior to its use at a job location constitutes a Severity Level V Violation (Supplement I).

MAINE YANKEE RESPONSE Upon receipt of the violation, Maine Yankee performed the following immediate, corrective actions.

The diagrams were removed from the job location and the file that contained the uncontrolled prints was removed from the work place and is no longer available for use in work packages.

MaineYankee United States Nuclear Regulatory Commission Page Two Attention: Document Control Desk HN-88-18 responsible for assembling and approving work packages to discuss this violation and the use of uncontrolled documents in work packages.

Additionally, a document discussing this violation has been distributed to all affected personnel under the required reading program.

Each individual must acknowledge that they have read and understood the contents of the document. Through these actions, we have stressed the importance of using documents reviewed for accuracy against controlled documents when performing work in the plant.

Maine Yankee also plans to take some additional actions to improve administrative controls over documents used in work packages.

To ensure that drawings used in DP/R0 work packages are acceptable, the work package l

assembly checklist in the DR/R0 work package assembly guideline will be modified to include a specific check off for verification of the drawings' accuracy. The revised DR/R0 work package assembly guideline and personnel training on its use will be completed by March 31, 1988.

This action should provide for added confidence that documents used at a job location have been reviewed for accuracy.

Maine Yankee achieved full compliance on December 7, 1987, when the documents were removed from the job location.
