Source Leak Test Failure Notification Report This is a leaking source exceeding leak test requirements notification report to the Nuclear Regulatory Commission (NRC) concerning the identification of an 8 mci Ni-63 source located at Battelle Memorial Institute (BMI) in Columbus, Ohio, under Licens' No. SNM-7, which exceeded 0.005 uCi loose contamination on its leak test. This five day reporting is made in accordance with SNM-7, Amendment 11, Condition 31.F. No personnel expo.sures occurred as a result of this leaking source.
A semiannual source leak test unducted at 11:00 a.m. on June 18,1997, identified a leaking 8 mci Ni-63 source housed in an Electron Capture Detector (ECD) which was removed from a Gas Chromatograph and was in storage. The leak test detected 0.03 uCi of activity, which is greater than the reporting limit of 0.005 uCi set forth in BMI's materials license, SNM-7, Amendment 11, Condition 31.F.
The 8 mci Ni-63 source, Model Number 021972-00, Serial Number A2716, manufactured by Varian is a solid fo*rm, scaled source with an assay date of December 30,1981. The ECD which contained the source was removed from a Gas Chromatograph, Serial Number X-39691 on February 7,1997. A leak test was conducted on the Ni-63 source on February 10,1997 after removal from the Varian Gas Chromatograph with satisfactory results.

Upon determining the source exceeded the leak test limit, it was bagged, tagged as radioactive material, sealed and transferred to Radiation Safety Services control awaiting return shipment to the manufacturer. The Radiation Safety Officer (RSO) was notified and a follow-up contamination survey was conducted of the area surrounding the source. No contamination was found in the surrounding area. No yrsonnel were exposed to loose surface contamination or any detectable radiation levels during this event.
