EVALUATIONS AND CONCLUSIONS REGARDING YOUR RTPLY TO THE NOTICE OF NONCONFORMANCE (INSPECTION REPORT 99900081/94-01)
Dear Mr. McAlees:
I am responding to your September 28, 1994, reply to the Notice of Nonconformance that was issued on August 29, 1994, by the U.S. Nuclear Regulatory Commission (NRC).
The Notice was issued pursuant to the NRC inspection conducted February 28 through May 13, 1994, at the engineering and manufacturing facility of Siemens Power Corporation - Nuclear Division (SPC-ND) in Richland, Washington.
The inspection was conducted to evaluate SPC-ND management, staff, and quality program and its implementation related to the supply of pressurized-water reactor core reload designs and analyses, fuel rod fabrication and fuel bundle assemblies, and other fuel-related services to the U.S. nuclear industry.
This inspection was also conducted, in part, as a followup to the NRC's augmented inspection team investigation of the misconfigured fuel bundle assemblies that SPC-ND supplied to the Carolina Power and Light Company (CP&L) plant, H. B. Robinson Unit 2 (HBR2).
The Notice of Nonconformance described three instances in which the implementation of your quality assurance program failed to meet certain NRC requirements.
Specifically, SPC-ND failed to (1) establish adequate measures to assure proper and timely closure of corrective actions that resulted from internal audits conducted by the quality assurance (QA) organization; (2) prescribe certain activities that affect quality, assure that activities that affect quality are accomplished in accordance with instructions, procedures, or drawings, and assure that instructions, procedures, or drawings contain the appropriate acceptance criteria; and (3) train employees performing certain critical fuel bundle assembly operations.
The team also identified SPC-ND's weaknesses in (1) identifying, for its neutronic engineering error evaluations, either the cause of the error or the corrective actions that 3
prevent recurrence; and (2) revising the neutronics guidelines and safety analysis procedures for pressurized-water reactors to represent the methodologies currently used by SPC-ND.
Your reply to the Notice nf Nonconformance, and the corrective actions taken i
by SPC-ND appear to be adequate.
No further response regarding the Notice of Nonconformance is required by SPC-ND.
We will review the implementation of SPC-ND's corrective actions taken to prevent recurrence during a future inspection.
