CP-87-109 (FINAL REPORT)
Gentlemen:
On October 5, 1987, we verbally notified your Mr. H. S. Phillips of a deficiency involving inappropriate deficiency documentation. After further evaluation, we have conservatively determined that this deficiency is reportable under 10CFR50.55(e).
The required infonnation follows.
DESCRIPTION L
l This deficiency involves documents that identify problems, request l
clarification, reconinend changes, or request action for engineering resolution, which were inappropriately used as design change documents, or identified potential nonconforming or deficient conditions.
These conditions l
may have resulted in the implementation of design :hanges which had not received appropriate design review and nonconformances or deficiencies not being resolved in accordance with established programs.
This deficiency was caused by a general lack of procedural requirements for utilization of various documents that had. requests for responses from engineering.
This condition appl.2s to documentation requiring respcnses from earlier TU Electric engineeriiig orgartizations (TNE, CPPE) prior to form lization of the Comanche Peak Engineering (CPE) Organization.
SAFET) IMPLICATIONS Due to the extensive evaluation required and the number of deficiency documents (Nonconformance Reports, Deficiency Reports and Design Change Authorizations) generated during the evaluation, we have conservatively concluded that this issue is reportable under 10CFR50.55(e).

V TXX-88010 February 11, 1988 Page 2 of 2 CORRECTIVE ACTION The evaluation for this issue is being conducted under Corrective Action Request (CAR)87-073.
The final resolution of CAR 87-073 provides an in-depth report on the corrective / preventive actions taken to resolve this issue.
To briefly summarize the CAR report,'a review was conducted of-historical types of documents with which engineering was involved.
Discrepant documents identified during this review had Nonconformance Reports (NCRs), Deficiency r
Reports (DRs) or Design Change Authorizations (DCAs) initiated on them, as appropriate, to identify and track corrective actions.
Actions taken to resolve these deficiency documents will be completed by December 31, 1988.
Most of the type of documents reviewed are historical documents and are no longer in use.
The design control program has been strengthened through the implementation of revised procedures. The actions necessary to prevent the previous problems from recurring are now in place and are discussed in detail in the final report to CAR 87-073.
Documentation supporting the problem identification phase (CAR 87-073) is available onsite for your Inspectors review.
Documentation supporting the completion of the corrective actions (NCRs DRs, DCAs) will be available at the site after January 31, 1989.
