Subject:
Modification No. 4 to Task Order No. 22 entitled. " Individual Plant Examination (IPE) Reviews - Internal Events - Human Factors Only (Quad Cities)" under Contract No. NRC-04-91-069 The purpose of this modification is to extend the period of performance of the subject task order from September 29. 1994 through March 29, 1995, at no additional cost or obligation to the Government.
The extension is needed to allow NRC to perform its internal reviews and provide comments to the contractor.
Accordingly, the task order is hereby modified to read as follows:
The first sentence of Paragraph 2 of the definitization letter is deleted in its entirety and the following is substituted in lieu thereof:
" Task Order No. 22 shall be in effect from September 30. 1993 through March 29, 1995, with a total cost ceiling of $18.958.00."
A summary of obligations for this task order, from award date through the date of this action is given below:

$18.958.00 Cumulative Total of NRC Obligations:
$18.958.00 This modification obligates FY94 funds in the amount of $-0.
All other terms and conditions of the subject task order remain unchanged.
