Modification No. 5 to Task Order No.1 Entitled "Public Comments for Patient Release Criteria Rulemaking" Under Contract No. NRC-04-93-058 This modification No. 5 is issued to extend the period of performance from 1
June 30, 1994, to May 31, 1995, at no additional cost or obligation to the Government. This extension is necessary to allow for comment evaluation and development of the final rule. The task order is therefore modified as follows:
The first sentence of Paragraph 3 of the definitization letter is deleted in its entirety and the following is substituted in lieu thereof:
" Task Order No. I shall be in effect from February 26, 1993, through May 31, 1995, with a total cost ceiling of $55,451.00."
A summary of obligations for this task order, from date of award through the date of this action, is given below:
Total FY93 Obligations: $54,775.00 Total FY94 Obligations: $
676.00 Cumulative Total NRC Obligations:
$55,451.00 All other terms and conditions of the subject task order remain unchanged.
Please indicate your acceptance of the Modification No. 5 to Task Order No. I by having an official authorized to bind your organization execute three copies of this document in the space provided and return two copies to the Contract Specialist.
You should retain the third copy for your records.
