SUBJECT: NRC INSPECTION REPORT 50-298/94-12

This refers to the special inspection conducted by Mr. R. A. 'Kopriva, and the

other inspectors identified in the attached report, of this office on

March 17-23, 1994.

The inspection included a review of activities authorized

for your Cooper Nuclear Station facility. At the conclusion of the

inspection, the findings were discussed with you and those members of your

staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The purpose of this inspection was to determine

whether activities authorizcd by the license were conducted safely and in

accordance with NRC requirements.

During review of this event, the inspectors identified a number of concerns

related to the performance of the Problem Resolution Team, which was created

by licensee management to review this event.

The conclusions regarding the

,

performance of the Problem Resolution Team included items such as the failure

to review all available data and collect all information prior to determining

the probable cause for the event, the failure to establish a detailed sequence

of events in order that the event could be fully analyzed and understood, and

not performing a bounding analysis of the pressure transient.

In addition,.

the inspectors noted several anomalies that your staff could not explain, nor

was there indication from your staff that the issues identified by the

inspectors were being given the appropriate consideration.

The inspectors

also noted that the members of the Problem Resolution Team had not received

formal training in the~ area of event analysis. These identified' weaknesses in

the performance of the Problem Resolution Team are of particular concern since

similar. weaknesses were also-identified during our review of the plant

transient that occurred on March 2, 1994, as documented in NRC Inspection

Report 50-298/94-10. The observed level of performance of.your staff, during

review of these events, is an indication that management should-review the

implementation of the processes used for event review and should take the

appropriate corrective actions to ensure that management expectations are met
