COMANCHE PEAK STEAM ELECTRIC STATION (CPSES)
DOCKET NOS. 50-445 AND 50-446 UNQUALIFIED LIMITORQUE ACTUATORS SDAR: CP-87-132 (INTERIM REPORT)
Gentlemen:
On December 18, 1987, we verbally notified your Mr. Shannon Phillips of a potentially reportable deficiency involving unqualified limitorque actuators.
After further evaluation, we have conservatively concluded this deficiency is reportable under the provisions of 10CFR50.55(e).
The required information follows.
Description During review of the equipment qualification for limitorque actuators, the following deficiencies were identified, a.
Unqualified terminal blocks have been supplied / Installed in both power and control applications, and b.
Unqualified nylon insulated wire connectors have been installed to connect dual voltage motors.
As a result of these deficiencies and other possible concerns noted in IE Notices 80-03 and 86-71, field validation efforts were initiated (per Field Verification Method (FVM) TE-FVM-EQ-047, "Limitorque Actuator Walkdowns")
which resulted in the identification of the following additional deficiencies.
a.
Unidentifiable or damaged wire inside the limit switch compartment, b.
Cracked limit switch rotors, and 1
c.
Motors inside containment which are inadequately drained (T-drains f
at the lowest point of the motor).

.Page 2 of 2 Due to the number and type of these deficiencies, we have conservatively determined that the limitorque actuators are not environmentally qualified and capable of perfonning their intended safety functions.
This deficiency is the result of inadequate vendor manufacturing controls.
The possibility of these deficiencies extends to all Limitorque Actuators.
Safety Implications Limitorque Actuators are used extensively throughout the plant in safety-related valve applications.
Failure of certain actuators could adversely affect safe-shutdown capability and/or the ability to mitigate the consequences of an accident.
Corrective Action t
All Class 1E Limitorque Actuators will be examined and corrected where required under FVM TE-FVM-EQ-047.
To preclude reoccurrence of these concerns,

the supplier has been notified and design procedures have been developed to t
assure equipment is qualified in accordance with the design criteria.
