Box 195 Wayne, PA 19087-0195 SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-352/97-03, 50-353/97-03 AND NOTICE OF VIOLATION Dear Mr. Smith: From March 30,1997 through May 27,1997, the NRC completed an inspection at the Limerick Generating Station, Units 1 & 2. The enclosed report presents the results of that inspection. During the 8-week inspection period, the conduct of activities at Limerick 1 & 2 was generally characterized by safe and conservative operations. An inspection, conducted to review your investigation into the improper control of vital area and locked high radiation area keys, found that the investigation was thorough and complete and that appropriate compensatory measures were promptly implemented to mitigate any vulnerabilities until the potentially comp.omised locks could be changed. Based on the results of this inspection, the NRC hac determined that violations of NRC requirements occurred. The violations are cited in the enclosed Notice of Violation (NOV) and the circumstances surrounding them are described in detail in the subject inspection report. The first violation is of concern because several grand ma. ster keys were not properly controlled. Although it does not appear that the keys were misused, the potential was clearly present for several years. The second violation concerns an instance in September 1995, where a procedure was temporarily changed without a prior safety review, to avert a plant shutdown when primary water chemistry levels exceeded the procedural limits. Although the safety consequences were low for this instance, the NRC was concerned that the change was made using the wrong process, which bypassed the prior review required of an intent change to the procedure. The NRC has concluded that information regarding the reason for the violations, the corrective actions taken or planned to correct the violations and prevent recurrence is already adequately addressed in this inspection report, and in NRC Integrated Inspection Report 50-352/97-01, 50-353/97-01 for the radiological aspects of the key control issue.
