pSC Response:
a.
Current receipt inspection requirements are set forth in procedures ENG-16, MRIM-1, and Q-4 for all safety-related and, as required, non-safety-related fastener purchase orders.
It should be notad that PSC does not currently purchase non-safety-related faAteners for stocking purposes but instead for specific applications only.
Characteristics examined by QC receipt inspection for safety-related fasteners are d aignated by the purchase order and normally include dimensional verification, documentation review, and other requirements applicable to the referenced ANSI, SAE, or ASTM specification, such as markings or hardness.
Non-safety-related fasteners are receipt inspected by QC personnel if requested.

Internal controls for storage and issuance of fasteners are delineated in procedure P-5.
Tags are used to identify and segregate materials in the warehouse.
Direct charge requisitions I
(DCR) are utilized to assure traceability for materials issued from the warehouse for use.
These OCRs reflect the work order (if applicable, or the location of work, or the use of the material), or station service request (SSR) number, and any other i
pertinent designators and are verified in the field by QC inspectors for the particular installation.
NRC Items 2-5:
2.
Select a minimum sample of ten (10) non-safety related fasteners (studs, bolts, and/or cap screws), and ten (10) safety-related f asteners (studs, bolts, and/or cap screws) from current, in use, stock.
The sample is to be obtained by the licensee with the participation of an NRC inspector.
Fasteners procured to meet l
the following chemical and mechanical properties are of interest:
A-193 grades B7, BS, and B16; SAE J429 grades 5 and 8; A-449; A-325 Types 1, 2 or 3; A-354 grades BB, BC, BD; A-490; A-320 LTM; A-307; A-563; or equivalent.

For the selected sample of fasteners in item 2, include a sample of typical nuts that would be used with each fastener (one-for-one).
In particular, nuts purchased to the chemical and mechanical specifications of A-194 are of interest.

Chemical testing shall be performed on all samples.
Mechanical testing shall be performed on each safety-related fastener.
Hardness testing shall be performed by a laboratory which the licensee has qualified for this type of tnsting and appears on the licensee's approved vendor list.
Testing performed shall be done in accordance with the requirements of the fastener's specification, grade, and class, and the test shall evaluate the ultimate tensile strength, hardness and chemical properties as required by the fastener's specification, gr.sde, and class.
Each sample shall be tagged with the sample's 10 number,

The results of all tests, together with supporting information, are to be reported to the NRC utilizing the format shown in 3
Attachments 1 and 2 of this bulletin.
Include the names and addresses of suppliers and manufacturers of safety-related fasteners and, to the extent possible, of non-safety-related fasteners.
For any fastener found out of specification, provide an evaluation of the safety significance including consideration of the most limiting application.
PSC Response:
Sample and test information required by Items 2-5 is represented in Attachments 1 (safety-related fastener and nut data sheets), 2 (safety-related fastener and nut test data), 3 (non-safety-related t
f astener and nut data sheets), and 4 (non-safety-related fastener and nut test data).
Samples were chosen with the assistance of the NRC Resident Inspector and were tested by Coffer Laboratories, Inc.,
a PSC-approved vendor.
j All safety-related samples met the required specifications with the following clarification:
sample 520SR did not meet the thread number i
required by ASTM A354 Gr.
BD, with 16 threads per inch (10 specified), and was documented on Nonconformance Report 88-009.
Upon further investigation, it was found that ANSI B18.2.1 was also specified in the purchase o'. de r, which allows alternate thread numbers while still meeting testing requirements of ASTM A354 Gr. BD.
Therefore, Nonconformance Report 88-009 was dispositioned "use-as-is" with no further evaluation required. Also, sample S12SR was not able to be tensile tested due to the size and shape of the head, i.e., the fastener was not long enough to be machined and the standard tensile machine fixtures / attachments are not compatible with this head shape.
Sample 512SR was from the same manufacturer and purchase order number as sample 511SR (also ASTM A193 Gr. BSM), which met all requirements.
The chemical analysis for sample 512SR was satisfactory, and hardness testing was performed with an approximate tensile strength value obtained that met specification requirements.
J Non-safety-related fasteners were tested and then evaluated to the most applicable specification that they represented due to the limited traceability of non-safety-related fasteners.
Nonconformance Report 88-011 was issued because sample 56N was marked as representing ASTM F593, alloy 304, but chemical analysis revealed that it was actually alley 305.
The manufacturer is unknown; Nonconformance Report 83-011 was dispositioned "use-as-is" because the mechanical properties are the same for these alleys, according to ASTM F593, and they will be used for non-safety-related applications only.

Based on the results of the testing and review of current procedures, describe any further actions being taken to assure that fasteners used in the plant meet the requisite specifications and requirements and that the operability of safety-related plant components is not affected.
