Armco, Incorporated Advanced Materials Division Docket No. 99900268/78-02 NOTICE OF DEVIATI0tl Based on the results of an NRC inspection conducted on August 7-10, 1978, it appeared that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:

A.

The Armco Advanced Material Division letter dated March 15,1978, in response to Notice of Deviations, Item A, enclosed with the NRC inspec-tion report cover letter dated February 23, 1978, indicated that all active Mill Orders would be revised by March 18, 1978, to properly identify the operations with the appropriate procedure numbers as required by instructions in the QA Manual.

Contrary to the above, during the current inspection it was found that the four active Mill Orders, previously identified, i.e. No. SN7-9738-2, 1P, No. SN7-9738-4, 4P, No. SN7-9738-5, 1P, and No. SN7-9738-5, 4P, had not been revised to properly identify the appropriate procedures numbers.

B.

Criterion V of Appendix B to 10 CFR 50 requires that activities affect-ing quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.

Welding Procedure No.12 dated March 15, 1973, used in tack welding both nuclear and non-nuclear stainless steel piping, required the use of 3/32" diameter filler wire and an Argon weld shielding gas flow rate of 20-30 CFH.

Welding Procedure No. 5, used in automatic submerged arc repair welding of an outside long seam defect cavity in a 12" 0.D. x 1.25" wall, T-304 pipe for Mill Order SN7-9871-1, 3P, required that an amperage range of 460-520 amperes be used with a 3/32" diameter filler wire.

Contrary to the above, the following conditions were observed:

1.

During the tack welding of a long seam in a 12" 0.D. x 0.875" wall, T-304 pipe, using procedure No.12, the tack welding was being performed without filler rod and using Argon shielding gas ficw rate of 45 CFH.

2.

During the repair welding of the defect cavity in the pipe weld for the above T-304 pipe for Mill Order SN7-9871-1, 3P, using Procedure No. 5, the amperage range being used was observed to be 420-450 amperes instead of 460-520 amperes, as required.

Criterion IV of Appendix B.to 10 CFR 50 requires that measures shall be established to assure that design basis and other requirements, which are necessary to assure adequate quality, are suitably included or referenced in the documents for procurement of materials.

The QA Manual, paragraph S.2.2, states in part, " Material orders shall be entered on a Purchase Order Form... and shall contain the following statement:

' Materials are ordered according to ASME Code,Section III requirements, and are subject to inspection before shipping'."

Contrary to the above, the following purchase orders for materials did not contain the above statement:

1.

P.O. No. VRM 292794, dated April 26, 1978.

2.

P.O. No. VRM 291032, dated January 18, 1978.

3.

P.O. No. VRM 292752, dated May 22, 1978.

D.

Criterion XII of Appendix B to 10 CFR 50, requires that measures be established to assure that measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits.

The QA Manual, Section 11, paragraph 11.2.5, states in part, "...

An ' Inspection Tag',..., is attached to each machine indicating the last calibration date,.... "

QC Procedure No.12 requires that the Model 40 Foxboro Controller / Recorder on the continuous Bay Hydrostatic Tester must have a dated instrument calibration label and a letter of certification.

Contrary to the above, the following conditions were observed:

1.

The Severn Gage, used for measuring delta ferrite, was not controlled by inclusion in the calibration program, as required by Criterion XII.

2.

The Inspection Tag, SN-0035, on the Hun 4 automatic welding machine showed a calibration date of September 2, 1977, instead of the recent calibration date of March 8,1978, as required by the QA Manual.

3.

The Foxboro Recorder Model 40, located on the continuous hydro-static tester, did not have a dated instrument calibration label, nor a contractor's letter of certification, as required by QC Procedure No.12.
