Investigation on: October 14-15, 1980 (Report No. 80-01)

Areas Investigated: Investigation of possible exposure to employees f.n the company's facility during radiography of welds for welder certification and whole body overexposures to personnel involved in on site operations. The investigation involved five (5) hours on site by one (1) NRC investigator.

Results: The licensee conducts the radiography of welds for welder certification where exposure to personnel in the offices is below the limits set for populated areas. The overexposure to the radiographers occurred dur1ng the month of July when they operated a model 800 x-mas pipeline crawler 200 KV x-ray machine.

No licensed material was involved in any of the exposures for the month of July 1980. Personnel were restricted from any further exposure since July 1980 and the date of the investigation.

No items of noncompliance was found.

REASON FOR INVESTIGATION q

l The NRC Region IV office received a letter. of complaint from a former employee I on October 6, 1980. The contents of the letter alleged improper precautions while radiographing test welds to certify welders at the facility and high  !

exposures to personnel' working at job sites that were in excess of the limits in 10 CFR 20.101(b)(1). '

SCOPE OF INVESTIGATION On October 14-15, 1980, a Region IV inspector visited the licensee's facility for the purpose of investigating the reported conditions. The inspector reviewed relevant licensee records and interviewed licensee employees.

CONCLUSION The licensee conducts the radiography tests of the welds for welders certifica-tion in a prudent and safe way. The licensee's TLD badge records show three personnel received in excess of 5 rem exposure during the month of July 1980.

Investigation by the inspector and' discussion with personnel involved in the incident disclosed that the exposure occurred in Texas and was due exclusively to a crawler x ray device. The personnel thus exposed were taken off all radiographic exposures. The TLD badge results since the exposure as well as records of work indicate that the employees have received a 0 occupational radiation exposure up to the date of inspection. Since the exposure was pure x-ray it appears to be out of our jurisdiction.

The device used which resulted in the overexposures is being evaluated by the Oklahoma Radiological Safety personnel. The manufacturer will be requested by the state of Oklahoma to evaluate and install a safety switch or other modi-fication to prevent a recurrence.

DETAILS

1. Introduction s A. Responding to a letter of complaint NRC Region IV conducted an

. investigation of the Tulsa, Oklahoma facility of Peabody Pipeline Testing Company. The complaint alleged that personnel were being exposed to radiation in unrestricted areas and that personnel had received excessive exposures. The investigation included interviews with the personnel from Peabody Pipelin- and an examination of reports and records.

3. Investigation A. Exposure to radiation in unrestricted areas.

The company perform radiography on test welds at the far end of the building (see sketch). The purpose of the tests are to certify welders. Such radiography is conducted on a nonrcutine schedule.

When conducted the end of the pigtail is placed in the center of the pipe section which is located inside a three quarters inch thick section of pipe. The test pipe is three eighths inches thiex. Both pieces of metal reduce the exposure significantly. The distance to the office area reduces the levels to nominal levels. A worst cause exposure was conduteted by the company after working hours. The maximum amount received for a two minute unshielded exposure was 2 mrem for a 44 curie iridium 192 source (see sketch). The dosimeters were placed at about three and a half feet (3 ) above the gr'ound. Under these conditions it will not be feaseable to receive an exposure above the weekly limits received in an unrestricted area. The

activity occurs two or three times a month a small number of welds.

B. Overexposure of Personnel The' exposures received by R. Coe of 8.060 rem, V. Van Burnt of 6.070 rem and by B. Freeman of 6.850 rem occurred during the TLD dosimeter periods of July.1 through July 31, 1980. The date of occurrence was July 24, 1980 in Shamrock, Texas while work was I being performed for the National Gas Pipeline Company. The unit causing the 2xposure was a X-Mas model 800 pipeline crawler 200 Kv x-ray machine. It was determined from interviews and examination of the exposure and work records that all the exposure received

.for the month was from x-ray exposure. Records of TLD badges '

reviewed for the personnel remaining with the company were 0 for the months of August and September 1980. The RSO had restricted the activity of these' personnel to non radiation jobs. Information received from the radiographers confirms the findings.

4. Management Discussion The results of the investigation, findings and the conclusions reached were discussed with the personnel indicated in paragraph 2.

Radiation Incident Report 9/12/80 NAMES OF PERSONS INVOLVED: Bill Freeman, Ronnie Coe and Kermit VanBrunt DATE OF INCIDENT: July 24, 1980 ,

TYPE OF SOURCE: 200KV X-Ray machine While working on an x-ray machine, making adjustment for 30" pipeline, a gas generator was started for a power source to the x-ray tube. The power switch was accidently turned on and the x-ray machine was producing x-rays. These men told me the x-ray machine was set for a 15 second sxposure. They then looked at their dosimeter and found it to be off scale. They did not notify the Tulsa office.

When the radiation dosimeter report was returned to the Tulsa office from R. S. Landover, Jr. & Company for the month of July, we then noticed these three men had an over exposure on their July TLD report. We then notified these men about the over exposure and requested a written statement from each of them stating what had happened.

We have laid these three men off until further investigation is made.

They will not be permitted to return to work for Peabody Pipeline until they have successfully been re-tested on radiation safety.
