SUBJECT: NRC INSPECTION REPORT NO. 50-440/97004
Dear Mr. Myers:
On May 2,1997, the NRC completed a routine safety inspection at your Perry 1 reactor
facility. The purpose of the inspection was to determine whether activities authorized by.
the license were conducted safely and in accordance with NRC requirements. During this
period routine resident inspections were performed. At the conclusion of the inspection,
the findings were discussed with members of your staff. The enclosed report presents the
results of that inspection.
During this 6-week inspection period an unexpected partial reactor trip signal and partial
containment isolation occurred with the plant at full power. Overall, operations performed
well, with specific improvements noted in many shift and evolution briefings. Some long
standing equipment problems contributed to additional operation distractions, especially in
the residual heat removal system. In addition, poor procedures were involved in an
operational transient which caused reactor recirculation flow control values to partially
close.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and the enclosures will be placed in the NRC Public Document Room (PDR).
