I NOTICE OF DEVIATION Based on the results' of an NRC inspection conducted on June 27-29, 1978, l

.it appeared that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:

A.

Criterion. XVII of Appendix B to 10 CFR' 50 states in part,. "...

Consistent with applicable regulatory requirements the applicant shall establish requirements concerning record retention,'such as duration,' location, and assigned responsibility."

QA Manual,Section XVII, Quality Assurance Records, Revision 3, dated July 22, 1977, states in part, "... Objective evidence records are filed and maintained in facilities that provide adequate protection...."

l Contrary to the above, objective evidence records are filed and maintained in a standard storage cabinet that does not provide

. adequate protection for the only file or source for certain project / equipment documentation. - (Examples: original tracing, R&D qualifications-reports and design analysis and verification reports).

- Criterion XII of Appendix B to 10 CFR 50 states in part, " Measures shall b'e established to assure that tools, gages, instruments, and other measuring and testing devices used in. activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

t QA Manual,Section XII. Control of Measuring and Test Equipment, Revision 3, July 22, 1977, states in part, "... All calibrated i

equipment that has been subjected to conditions that could adversely affect the accuracy of indications,... or does not have a current and valid calibration sticker, shall be tagged imediately with an "Out of Service" tag.... Out of calibration or inoperative M&TE is withheld from use and segregated by QA until repaired and/or re-calibrated or replaced."

Contrary to the above, certain equipment marked "Out of Service" and without a calibration sticker or identification label was stored in the mechanical calibration test laboratory intermingled with pro-perly calibrated and serviceable test equipment (example: Pull Tester).
