We receive medical information en utility apoloyees only vtun they sooly for an operator or senior operator license pursuant to 10 CFR Part 55, Operators' License (cooy enclosed). As part of the applienHm the individual and his l
physician submit a completed Certificate of Medical History, Form PRC-396 (copy enclosed). ' Itis' Certificate is reviewed by our physician h mkes recomendatims resarding the applicant's physical and mental condition.
We have no record that indicates we have received an apo14e=Hm for an ooerator's license fra Mr. Evans, nor have we received any requests fra a utility for a ruling regarding his physical ability to perform licensed duties.
Aonarently, Mr. Evans was exac:ined by a physician employed by a utility. Se determination to refuse him encloyant was ade without any imut frm us.
W do nce know if the amination was a preemployment examinatirn or if it was one adrdristered in conjunction with their operator licensing progre.

In order for us to nake a determination regarding Mr. Evan's ohysical condi-tien, it utxild be necessary for us to review a eczpleted Cer*immte and a report of the results of an exa rination by an mild surgeon, including a 1cuer spinal X-ray.
